There are certain forms that are required for competition. If you have not mailed these forms to Karen by Wednesday you will need to bring a copy of them to the tournament. ALL competitors and coaches MUST have these forms completed to compete. If you have not done so all forms are attached for download at the bottom of this page.
For Each Team Member: FLL Consent, UH Release of Liability, UH Photo Release
For Each Coach, or Assistant Coach: FLL Consent and UH Release of Liability
Plus, the team should submit a Team Information Form with photo in the package at check-in and one at each judging station (you will need total of 4 copies).
Note: These forms should be saved to your computer, filled out when possible, then printed.
